The parent(s) of any child/ren attending the Breakfast and After School Club should have read and understood the Terms & Conditions before signing the following declaration.

In completing this form I confirm I have read the Saxon Primary School Breakfast Club and After School Club Information Booklet and agree to the terms and conditions. I understand these terms and conditions are reviewed and updated periodically on the school website and it is my responsibility to keep up to date with this.
